Please fill out the power of attorney, the Compensation Agreement and the questionnaire, sign the documents and send them to Nieding + Barth or TILP Rechtsanwälte

Please print out the power of attorney, the compensation agreement and the questionnaire. Should you not be able to open the documents, you will need to install Acrobat-Reader by Adobe.
Once you have printed out the power of attorney, the compensation agreement and the questionnaire, please complete them where required (by filling in your name etc.), and sign the power of attorney and the compensation agreement. Please send the originals to us by post; it will not be necessary to send them in advance via telefax.
Please note that we will be able to take action on your behalf only once the signed originals of the power of attorney and the compensation agreement have been provided to us.
